Do mistakes creep into projects ?

Imagine a conglomerate of organizations, involved in a new project. Mistakes creep into areas of responsibility of one of the conglomerate, ruining its reputation. It is cut off from the project, perhaps for 10 years. The mistakes it made, in its area of responsibility, start to diffuse into the project. The project and the member of the conglomerate who was cut off from the project because of their poor reputation, now seem to be "coming up with the same answers". So the formerly banished member of the conglomerate is seen as good value, and is able to rejoin the project.
