Does the the ionic diffusion causing action potential in an axon happen at the node of ranvier?

Yes. Impulses travel along myelinated nerve fibres which have Nodes of Ranvier by the action potential (which is basically a depolarised region of the fibre caused by an influx of Na+) "jumping" from Node to Node.
